Please note that your request must be placed before Monday at 10am each week. All requests received after the Monday 10am deadline will be processed the following week. Your employer has designated a weekly supplemental benefit amount. This is the amount that will be processed when you request supp benefits. You can always request less than this designated amount weekly but you cannot request more. Should you have any questions regarding this process, please call 1-800-724-4817 at extension 134 or 137.

Effective March 1st 2014, all direct deposit/paycard check stubs and monthly trust statements will be delivered electronically. Check stubs and statements will be available for retrieval on our website and in your online account/mobile app. You may opt out of receiving AND accessing check stubs/W-2's/statements electronically by submitting a waiver form. These forms are available on our website in the forms section (www.gmr-usa.com).
